I, the undersigned, as parent and/or legal guardian of the child identified on this form, do hereby give the Jewish Educational Alliance, Inc. permission to use names, photographs and videos of my camper. Photographs and videos may be used for promotional, fundraising, and educational purposes and programs to promote current and future JEA programming. I hereby release the Jewish Educational Alliance, Inc., and all of its employees, trustees, officers, directors, volunteers, students, agents and assigns from any and all liability and financial obligations related to the use of any photography or videos of me or my dependents.

• I understand that the JEA may, in its sole discretion, cancel events, scheduled practices, classes and/or games as it deems appropriate based on, but not limited to, inclement weather conditions, facility availability and/or conditions. No refunds or prorated refunds will be issued except at the sole discretion of the JEA. Refunds will only be issued for programs that are canceled due to an insufficient number of participants.

• In accordance with the JEA code of conduct, I understand that no foul language is permitted; no fighting, bullying or threat of assault is permitted.

• I understand completion of this course does NOT guarantee a position of employment within the JEA Aquatics Department.

• I understand that I am asking to participate in aquatic activities during the lifeguarding course. I acknowledge that these activities are not without risk. Certain risks cannot be eliminated without destroying the unique character of this activity. The same elements that contribute to the unique character of this activity can be causes of loss or damages to personal property, accidental injury, illness, or in extreme cases, permanent trauma or death. I understand that even though precautions have been taken by the Course Instructor, Aquatic Management Team, and the Lifeguarding Staff to provide safe aquatic conditions, I am responsible for my/my child's health and safety at all times.
